Our Senior Programs

ESAAA is pleased to announce the following Care Options.  

We have a program that will work for you!

Care Coordination

Care Coordination is a service that assists persons 60 years and older living in Accomack and Northampton counties to remain independent for as long as possible.  Care Coordination provides information, referrals, comprehensive assessments, as well as the coordination and monitoring of available services.

Care Transition

ESAAA/CAA is partnering with Riverside Hospital to reduce hospital re-admissions.  Our certified coaches will work with you so that you can remain healthy and independent.  

Caregiver Programs

ESAAA/CAA partners with Hospice, Riverside Hospital, Rural Health and the Alzheimer's Association to provide training and support groups for those caregiving for their loved ones.  Respite services are also available.

Commonwealth Coordinated Care Program
Commonwealth Coordinated Care is a combined Medicare and Medicaid plan that includes all the benefits you currently received under Medicare and Medicaid plus other additional benefits.

Consumer Directed Services
If you receive Personal Care Services through Medicaid, you can choose your own aide. If you have a friend or neighbor who you feel comfortable with, call to find out how they can be paid to act as your aide.

Home Delivered Meals

Providing frozen or shelf stable meals to qualified participants.

Long-Term Care Ombudsman Program

Advocating for quality care.

Nursing Home Diversion Community Re-entry

ESAAA/CAA works with nursing home residents who are able and desire to return to the community.  If you or a family member has been residing in a nursing home for 6 months or longer, we may be able to help.

Option Counseling
Have you had a major life change? Has your spouse died or have you been hospitalized? Our trained counselors can help you explore your next steps and help you plan for the future.

Outreach Programs

The Outreach Department is designed to provide services to persons/families seeking emergency assistance or services they are unable to obtain elsewhere. Outreach services are available to residents of Accomack and Northampton Counties.

Senior Centers with Congregate Meal Service

Socialization, recreation, with wellness and health education and a hot meal.


Helping navigate Medicare/Medicaid issues.
