Eastern Shore Agency on Aging
The Eastern Shore Area Agency on Aging is committed to serving the 60 and older population of Virginia's Eastern Shore. ESAAA focuses on the promotion of the maximum level of independence of persons 60 and older. The agency also works to ensure that older ESVA residents live as an integral part of society with dignity by providing access to the services and programs needed to make a healthier and more fulfilled life possible.
Through the Federal Older Americans Act, a network of over 670 Area Agencies of Aging exist in the US with the goal of improving the quality of life for older Americans. ESAAA is proud to manage and promote the following programs and services aimed at helping reach those goals:
Senior Centers with Congregate Meal Service - Socialization, recreation, with wellness and health education and a hot meal.
Long-Term Care Ombudsman Program - Advocating for quality care.
Home Delivered Meals - Providing frozen or shelf stable meals to qualified participants.
VICAP/SMP - Helping navigate Medicare/Medicaid issues.
Transitional Coaching - Developing new skills to better understand and manage your health condition.
Home Health Services - Personal Care, Homemaker Program, and Fee-for-Service.
Nursing Home Diversion Community Re-entry - Assisting nursing home residents return to the community .
Caregiver Coalition - Partnering with Riverside Hospital, Eastern Shore Hospice and Palliative Care, and Eastern Shore Rural Health.
Care Coordination - Informing 60 and older persons of available care, assessments, monitoring, and referrals.
Community Action Agency
Our Community Action Agency [CAA] is proud to provide Head Start Preschools as well as Low-Income Heat Energy Assistance Program to the Eastern Shore of Virginia.
CAAs have a long history of promoting self sufficiency through their 1000 plus community-based agencies in the US.
